Another man down for Ganna – he passes Giovanni Visconti.

A thumbs up for Dennis after the finish. Ganna has edged ahead by five seconds at the second check, meanwhile.

Ganna is coming up to the second checkpoint.

Černy won't get to enjoy the hot seat for very long, though. He has crushed it at an average speed of 47.5kph to set a time of 43:06, 50 seconds up on the CCC man.

De Gendt is third-fastest over the climb, just 10 seconds down on Ganna. Nice.

He's faster still at the third checkpoint. 30:37 puts him 36 seconds up on Černy, who comes across the line.

The Czech TT champion smashes Campenaerts' time! He's 49.65 seconds faster at 43:56. A great ride.

Ganna is just 1.59 seconds up on Dennis at the top of the climb! Phew, it looks like a close one...

Ganna is flying up the Muro. He's past minute man Jonathan Caicedo and two-minute man Giovanni Carboni as they head up the steep ramp.

Thomas De Gendt (Lotto Soudal) is off. He's a strong time trialist and good in the hills so it should be a good day for him.

Bjerg 34 seconds down on Dennis at the top of the climb. It's looking more and more like another intra-Ineos battle here.

"You kind of have to treat it as three time trials – get to the climb, do the climb, do a time trial afterwards," says Alex Dowsett about the course.

The Briton added that he didn't think there would be big gaps among the GC men, but thinks that João Almeida should extend his race lead.

World champion and double stage winner Filippo Ganna has set off. Another victory for him today?

Campenaerts is the new fastest man at the finish. He's the first man under 45 minutes with a time of 44:46.

21:47 for Dennis at the second check. He's blown away Černy by 28 seconds there.

45:14 for Kamil Gradek at the finish. The Polish champion tops the leaderboard for now.

Maglia ciclamino Arnaud Démare goes third fastest at the finish with a time of 47:27, over two minutes down on Brändle.

An update on Ciccone from Trek-Segafredo:

"As already known, Giulio suffered from COVID-19 at the beginning of September and his physical condition, although sufficient to be at the start of the Giro, could not be optimal. The difficult days just passed, with rain and cold, have then affected his clinical frailty and we believe that the choice to stop him is the most prudent to not overload an already tired physique," said team doctor Emilio Magni.

The team noted that all of Ciccone's COVID-19 tests have been negative. 

Bjerg sets off. Eight minutes until two-time stage winner Ganna starts.

Campenaerts eight seconds up at the third checkpoint.

58:31 for Molano at the finish. He finished over 13 minutes down after his crash.

Dennis 11:08 over the climb. That's the new quickest time there by a further 22 seconds.

Jan Tratnik (Bahrain McLaren), Mikkel Bjerg (UAE Team Emirates), Chad Haga (Team Sunweb) and Filippo Ganna (Ineos Grenadiers) all set off in the next 15 minutes.

Josef Černy has smashed it at the first check. He's at 11:30, 29 seconds up on Brändle. Dennis will be through there soon.

Dennis passes his minute man, Lorenzo Rota, after 5km. Meanwhile, Campenaerts has passed three men during his effort.

Rohan Dennis (Ineos Grenadiers) set off a few minutes ago.

Dowsett just misses out at the finish, six seconds down on his teammate.

Campenaerts is top at the second check now, 10 seconds up on Brändle.

Brändle fastest at the finish now with a time of 45:23. Now we wait for Dowsett.

Dibben is the first man to finish with a time of 47:52.

Campenaerts catches his minute man, Mathias Le Turnier.

Campenaerts is second over the Muro. He's just one second behind Brändle.

Giulio Ciccone (Trek-Segafredo) is out of the race. He's suffering from bronchitis. A big blow for Vincenzo Nibali.

Dowsett just two seconds down at the second check! It'll be a battle to the line between the ISN duo.

A bizarre crash for UAE Team Emirates sprinter Juan Sebastian Molano, who changes hand position heading into a corner and tries to take the apex too close, clipping the barriers on the inside and flying off his bike.

Time for Campenaerts to start his effort now.

Meanwhile, Brändle – who has gone through the second check fastest, too – has passed Vini Zabù men Simone Bevilacqua and Matteo Spreafico, who are drafting one another quite egregiously...

Dowsett is four seconds off his teammate over the top.

Brändle is the quickest so far over the top of the Muro. His time of 11:59 is 22 seconds faster than anyone else so far. Hodeg is the slowest at 13:18.

Dowsett has just set off now, too. All on TT bikes, of course – the Muro might be steep, but it's not worth the time loss of using a road bike along the 6km of flat roads beforehand.

Brändle is off now, as is Deceuninck-QuickStep sprinter Alvaro Hodeg.

The first riders are off – Jonathan Dibben, ISN's Guy Sagiv and Cofidis man Marco Mathis.

It's a rolling course today as you can see in the stage profile. However, there's a tough section early on as the riders tackle the Muro di Ca' del Poggio – a 1.1km wall at 12.3 per cent with sections reaching up to 19 per cent.

There's a time check at the top, after 7.4km, while two more follow at 17.1km and 25.1km.
